A recent editorial published by Win4RI physician leaders, Jim Fanale and Al Puerini, described innovations to improve the quality, access and cost of care for Rhode Island’s Medicare patients.
The build-out of this program – which arose out of The Margolin Group supported Win4RI partnership among Blue Cross and Blue Shield of Rhode Island, the Integra Community Care Network (the newly formed ACO network created at Care New England) and the Rhode Island Primary Care Physicians Corporation – has enabled providers from across the continuum of care ‘to work together to identify the biggest gaps in the delivery system.’ This work has led to the creation of the ‘Complex Care Management Program’ aimed at providing the care patients need in the right place, at the right time and in the right setting.
Drs. Fanale and Puerini noted that they would like to target initially 200 of the sickest, highest-risk, highest-cost BCBSRI Medicare patients with advanced illness that require special attention.
